The questionnaire was developed by researchers associated with the Université de Montréal and the Institut universitaire de gériatrie de Montréal.

Mapi Research Trust’s catalog of Clinical Outcome Assessments now includes a new questionnaire, developed by Dr Jesse Cedarbaum: The Amyotrophic Lateral Sclerosis Functional Rating Scale - Revised version (ALSFRS-R). This tool evaluates the progression of disability in patients with amyotrophic lateral sclerosis.
